Terms

Terms of service.

The agreement for using AllOneForm — what it is, what it is not, and who is responsible for what. Effective November 4, 2026.

The agreement

These terms are an agreement between you and AllOneForm ("AllOneForm," "we," "us") for the AllOneForm website and client intake platform (together, the "Service"). By using the Service — including by requesting early access, signing up, or signing in — you agree to these terms. If you use the Service on behalf of an agency, you agree for that agency as well and confirm you have authority to do so.

1. What AllOneForm is — and is not

AllOneForm is a client intake form for independent insurance producers. Clients complete a tap-first intake; the agency reviews one structured record and takes it into its own rater to quote.

AllOneForm is not an agency management system, a rater, or an insurer. It does not rate, quote, bind, or sell insurance, and it does not replace your rater or agency system — systems such as EZLynx remain your system of record for quoting and policies. AllOneForm is software. It does not give insurance advice, and nothing the Service produces is a quote, a coverage determination, or a promise that coverage is available or will be bound. Licensed producers at your agency are responsible for all advice, quotes, and placement decisions.

2. Early access

The Service is currently offered as an early access program to a first group of agencies. Features may change, be added, or be removed as the product develops. Accounts are provisioned during onboarding; your agency's intake address is assigned and confirmed at onboarding, subject to availability.

Pricing is discussed individually, on your walkthrough. No price is stated on the website or in these terms, and the early access terms for your agency are those confirmed with you at onboarding. During early access the Service is provided without a service-level commitment: we work to keep it running and to fix problems promptly, but uninterrupted availability is not guaranteed.

3. Accounts

Producer sign-in uses a username and password (email is kept on the account for invites and recovery). You are responsible for keeping your credentials confidential and for activity under your account. Each agency account is separate: a producer signed in to an agency can access only that agency's intakes and settings. Tell us promptly if you believe your account has been compromised. The agency is responsible for its users — who it authorizes, what they do in the backend, and the accuracy of the agency settings and branding it configures.

4. Acceptable use

You agree not to use the Service unlawfully, or to collect or handle information you have no right to collect or provide. You agree not to attempt to access another agency's data, another user's account, or any part of the platform you are not authorized to use. You agree not to interfere with the Service's operation or security, probe it for vulnerabilities except as we authorize in writing, or submit malicious code or automated abuse, including false form submissions. And you agree not to direct clients to your intake link in a misleading way, or present the intake as anything other than your agency's intake for quoting.

Agencies are responsible for obtaining and honouring the consents their own business requires. The intake's consent step and its consent records are platform features; they do not transfer the agency's own legal obligations to AllOneForm.

5. Agency content and client data

Your agency's client data belongs to your agency. AllOneForm claims no ownership of it. We process it only to provide the Service, as the Privacy Policy describes — including the one stated exception (optional end-of-intake product feedback, usable publicly with the client's first name only, under the consent wording shown to the client) and the platform's use of aggregated, de-identified statistics that cannot identify any client or agency.

Your agency's branding and settings — name, logo, colors, template wording, and the like — remain your agency's. You give us a licence to display and use them as needed to operate your agency's pages and records within the Service. The platform itself — the software, design, the AllOneForm name and logo, and all related intellectual property — belongs to AllOneForm; these terms give you no rights in it beyond using the Service.

Product feedback you give us about the Service — suggestions, comments on the product itself — may be used by us to improve the Service without obligation to you. This is separate from client feedback, which is governed by the consent wording shown to clients and the Privacy Policy.

6. The client-facing side

A client's intake is a request for a quote from your agency — nothing in the intake or the Service promises the client coverage, a rate, or a binding decision. Follow-up messages to clients are the agency's: the platform queues messages for the agency's review under the agency's own sending identity, and the agency is responsible for what it sends and for honouring clients' contact preferences and consent withdrawals. If you stop using the Service, client relationships remain yours. We do not contact your clients about leaving, and we never market to them.

7. Privacy

Our Privacy Policy describes how information is handled under the Service and is part of these terms. If these terms and the Privacy Policy conflict on a privacy question, the Privacy Policy governs.

8. Suspension and termination

You may stop using the Service at any time; contact us to close your agency's account. We may suspend or end an account for misuse of the Service, a security risk, or non-payment of any amounts agreed at onboarding, and we may end the early access program with reasonable notice.

While an account is active, the agency can access and export its client records from the backend. After an account closes, we will make the agency's client records available for export for a reasonable period on request, after which they may be deleted. Consent records may be retained as proof of consent, as the Privacy Policy describes.

9. Disclaimers and liability

During early access, the Service is provided "as is" and "as available," without warranties of any kind, to the fullest extent permitted by law. AllOneForm is not liable for insurance outcomes: quotes not offered, coverage not bound, rates, or any decision a carrier, agency, or client makes using — or instead of — the record the Service produces. Checking the record before quoting remains the producer's job.

To the fullest extent permitted by law, AllOneForm's total liability arising from the Service is limited to the amounts you paid us for the Service in the twelve months before the claim (or, if you have paid nothing, one hundred dollars), and neither party is liable for indirect, incidental, or consequential damages. Nothing in these terms limits liability that cannot be limited under Minnesota law.

10. Changes to these terms

We may update these terms. The current version will be posted at this address with its effective date. Material changes will be brought to account holders' attention by reasonable means; continuing to use the Service after a change takes effect means you accept the updated terms.

11. Governing law

These terms are governed by the laws of the State of Minnesota. Any dispute arising from the Service will be brought in the state or federal courts located in Minnesota, and both sides consent to that venue.
